Role Overview: The Sidmouth Hospice at Home service, funded by the charity and provided in partnership with the Royal Devon, offers integrated community-based specialist palliative care. The service aims to deliver personalized care to patients diagnosed as palliative by their GP, regardless of diagnosis or complexity, based on individual needs and wishes. It serves patients registered with a Sidmouth GP.
Requirements and Opportunities: Candidates should have experience in palliative care; community experience is desirable but not essential. There are opportunities for learning and professional development through reflective practice. You will join a professional, specialist team working alongside health and social care colleagues in both community and acute settings.
Service Details: The service operates 7 days a week, 365 days a year, with shifts from 08:00-16:00, 10:00-18:00, and 12:00-20:00. As a Clinical Nurse Specialist, you will be part of the on-call team providing overnight advice and visits, ensuring 24/7 specialist end-of-life care and support, helping to reduce unnecessary hospital admissions.

Salary and Benefits: The salary reflects the Agenda for Change 2025/6 rates, including a 3.6% pay award to be implemented later this year.
