Job description

Specialist Nurse for Looked After Children and Care Leavers

We are looking for experienced SCPHN HV / SN, someone who has a specialist children's nursing qualification or equivalent to join an expanding nursing team for Looked after Children within Leeds Community Health Trust. The posts are full time.

You will be working in our friendly and supportive nursing team caring for our children and young people who are looked after by Leeds city council, alongside their families and care givers, to maximise their health potential and address their health inequalities.

The successful applicant will be responsible for managing and monitoring the delivery and the health care of looked after children and young people, age 0-17. You will work both directly and indirectly with children, young people, families and carers to complete to improve health outcomes.

There is a hybrid approach to work within the team, working 50% of time from base and delivering support to looked after children across the whole of the West Yorkshire ICB area.

Main duties of the job

The role holder will be working with some of the city's most deprived children and young people and be able to demonstrate an understanding of the challenges this brings to practice.

Multi-agency and professional working is essential to this role, so you will need to possess excellent communication skills, be self- motivated, and have a keen desire to improve the outcomes for looked after children and their families.

Day to day working will need excellent organisational and planning skills, as you will have responsibility for managing your own diary.

As a team we want to encourage development within the role and there will be the opportunity to develop expertise in given areas.

Applicants will need to have at least 2 years post registration experience and be able to demonstrate knowledge and experience in the field of looked after children and safeguarding.
