Job description

Central London Community Health NHS Trust (CLCH) is seeking to recruit a Specialist Nurse for Looked after children in Merton. This is an exciting opportunity for a compassionate and dynamic practitioner to join our children's 0-19 team.

We are looking for a practitioner with experience of working with vulnerable and complex children and young people. The individual will be able to work in a collaborative manner and be able to liaise professionally with other agencies working with Looked after children and young people i.e. social care, health partners and foster carers.

The post holder will be supported to influence the Looked after children's nursing agenda in Merton. The ability to work independently is essential and the post holder will be required to travel to other boroughs across the country to support looked after children who are the responsibility of Merton Local Authority and placed in other boroughs. You will be a highly motivated individual who confidently supports and delivers a responsive and inclusive Looked after children's nursing service.

This individual will be joining a professional team who is embedded within the 0-19 children's service in Merton. The role will work alongside health visitors, school nurses, children's therapy and local health partners in the community and the acute sectors.

Main duties of the job

• To maintain a clinical caseload and assess health needs with a child centred holistic framework in line with the 'Framework for the Assessment of Children in Need and their families'. (2000)
• Ensure effective implementation of healthcare plans for individual children
• To work in partnership with the Named Doctor and Named nurse to undertake holistic health assessments for all children entering care
• In partnership with the Named Doctor, Named nurse and GP's, develop individual Health.

Qualifications

• Registered Nurse with current UK NMC registration
• Health Visiting / School Nursing professional qualification, with entry onto the 3rd part of the NMC Register
• Evidence of ongoing professional development in Safeguarding Issues
• Minimum of two years post registration experience working in the acute sector or in the community
• Family Planning qualification

Experience

• Demonstrable post qualification experience of working with children & young people in a range of settings
• Experience of working with highly vulnerable children & young people
• Carrying out assessments of a CYP's development and identifying where further investigation, assessment or escalation may be required
• Formulating care planning & intervention, directing other professionals to support achievement of the CYP's outcomes Working in partnership with vulnerable children & young people Multi-agency liaison & collaboration, with respect to complex children's cases
• Able to demonstrate relevant effective learning from previous experience
• Experience of working with Looked After Children and young people
• Experience of multi-agency working arrangements
• Working with client groups from diverse cultural backgrounds. Ability to empathise with vulnerable children, young people and their carers and families
• Experience of consultations with young people

Skills

• Awareness of national and local policies and Statutory Guidance with respect to LAC
• An understanding of the public health context of supporting LAC Ability to work with CYP from diverse cultures, Asylum Seekers & those who are 'Hard to Reach'Has participated in audit and can advise how this has transferred into practice
• Excellent IT, verbal and written communication skills
• Good communication & interpersonal skills.
