Job Overview

An exciting opportunity has arisen in the integrated looked after children team to join us as a Specialist Nurse for Looked After Children. The integrated looked after children's team support children/young people and their carers to meet any identified physical health, mental health and emotional well being needs . Looked after children are a particularly vulnerable group of young people that are often difficult to engage and although they have many of the same health issues as their peers, the extent of these is often greater because of their past experiences

As a Specialist Nurse you will be required to enable timely completion of review health assessments for looked after children in line with statutory guidance and provide specialist advice to carers and staff to improve health outcomes for this hard to reach group of children and young people . You will use evidence-based practice to deliver high quality care and signposting to most appropriate services to ensure that the children and young people are supported to make healthy lifestyle choices.
