Are you ready to make a real difference in the lives of frail patients? Join our dynamic and dedicated Frailty Intervention Team as a Band 6 Nurse!

About the Role:

As a vital member of our multidisciplinary team, you will play a crucial role in delivering top-notch patient care. Your efforts will focus on implementing evidence-based improvements to identify frailty early and mitigate associated risks.

Key Responsibilities:
  1. Proactive Case Finding: Identify patients living with frailty within urgent and emergency care settings.
  2. Comprehensive Geriatric Assessment: Develop thorough assessments to understand each patient's unique needs.
  3. Discharge Planning: Initiate and manage discharge plans, including social care packages, fast track, end-of-life care, and rapid discharges, all aimed at avoiding unnecessary hospital stays.
