Role specifics
We have two Band 6 Specialist Nurse (Community) posts available—one full-time and one part-time (30 hours), both based in Exeter. Candidates should be highly experienced community nurses with current clinical competencies, knowledge of primary care, community nursing, social care, and integration. The successful candidate will oversee designated caseloads, demonstrate clinical competence, undertake assessments of patients with complex needs, develop care plans, and monitor standards of care. Leadership involves maintaining patients in their preferred place of care, supporting independence, and ensuring high standards through team management and effective documentation, especially using EPIC systems. The role includes triage responsibilities, leading weekend coordination, and supporting multidisciplinary meetings.
Applicants must have a postgraduate/post-registration Clinical Diploma in Advanced Clinical Practice or equivalent, evidence of significant community nursing experience, mentorship qualifications, and experience supporting junior staff and students. Opportunities for further qualifications, including a Master’s level Specialist Practitioner qualification, are available.

Applicants should have a degree or be working towards one, with evidence of CPD and possibly leadership qualifications. A valid UK driving license is required.
Working Pattern: Full and part-time roles, typically 8:40 am to 4:30 pm across a seven-day week.

Salary: Reflects the Agenda for Change 2025/6 rates, including a 3.6% pay award, backdated if joining before the award implementation. Current pay scales are for 2024/25.
