Are you a motivated and compassionate SCPHN looking to make a real difference in the lives of vulnerable children and young people?

Locala Children Looked After and Care Leavers health team have an exciting opportunity for you to be part of a passionate multidisciplinary team committed to improving the health and wellbeing of some of the most vulnerable members of our community.

As a Specialist Nurse, you will play a pivotal role in supporting children, young people, and care leavers (from birth up to 25 years old) with a wide range of needs. You will work both independently and alongside other professionals and carers, building strong, supportive relationships to deliver high-quality, holistic care and assessments. You will often work with complex cases and must be confident in planning and delivering tailored interventions that respond to individual needs

Main duties of the job

Key Responsibilities

  • Positively engage with children and young people to promote good health and wellbeing.
  • Collaborate with professionals and carers to achieve the best outcomes.
  • Monitor and review clinical practice, engaging in reflective supervision to maintain high standards.
  • Identify and action health needs promptly, involving appropriate services and personnel.
  • Provide advice, training and support to carers and professionals to build their confidence.
  • Work in partnership with the Local Authority to support their corporate parenting responsibilities.
  • Promote flexible, responsive care and reduce barriers to access.
