Specialist Nurse Children in Care

Do you have a passion for promoting and improving the health for children and young people aged 5 to 18 in care? An exciting opportunity has arisen to join the Derbyshire Children in Care (CiC) Nursing Team working full time across our South Derbyshire caseload. We are passionate about ensuring children in care receive high quality health care plans within statutory time frames that support and improve their health outcomes. We are looking for a motivated and passionate Specialist Nurse to join our well-established, supportive and friendly team providing high quality statutory health assessments for CiC living in Derbyshire. Our base is in Clay Cross with nurses managing geographical caseloads across the county (this post is for the South Derbyshire caseload). We offer a variety of appointment types, mobile working and access satellite clinics.

The role covers all aspects of Children in Care including Unaccompanied Asylum-Seeking Children, Care Experienced Young People and children with complex health needs residing in residential settings. You will require experience of Safeguarding to actively participate in a Duty Day rota, which involves attending strategy meetings for children in care living in Derbyshire. The ability to maintain excellent record keeping and complete detailed reports and health care plans is a vital element of the role, alongside managing a caseload and working safely alone in the community. The role can be clinic based, office based and visiting children in foster and residential placements. We offer a full induction and competency package to support this role, alongside Safeguarding and Clinical Supervision

Main duties of the job

Complete Statutory Review Health Assessments within timescales.

Formulate robust, high quality Health Care Plans

Complete comprehensive child focussed reports capturing the voice of the child.

Work collaboratively with other agencies including Social Care, Policy, Safeguarding, Education and other health organisations.

Provide information and signposting to Multi-Disciplinary Team supporting Children in Care

Participate in Duty Days calculated pro rata.
