The Community Neurological Rehab Service has an exciting opportunity for a dynamic and skilled clinician to join our well-established team as a Specialist Neurological Nurse.
As a registered nurse, you will work as an autonomous practitioner, collaborating within an integrated multidisciplinary team, supported by the Clinical Team Leader. The successful candidate will be motivated, forward-thinking, enthusiastic, and committed to all aspects of the patient pathway.
You will manage a complex caseload, providing expert assessment and long-term condition management for patients with neurological conditions. Input will span from education at onset to mild and moderate complex disability. Care is delivered in patients' homes across Mansfield, Ashfield, Newark & Sherwood.
You will contribute to developing long-term condition management within community services and ensure that commissioned targets and KPIs are met.
Please note: applicants will need to pay for their DBS check, deducted from salary after appointment. The DBS cost is £26.40 (standard) or £54.40 (enhanced). We encourage enrolment in the DBS Update Service, with an annual fee of £16.
To provide specialist assessment, condition management, and access to interventions for patients with Long Term Neurological Conditions.
To offer timely, realistic, and occasionally intensive support within the community setting.
To demonstrate clinical leadership, providing support, advice, education, and supervision to junior staff and other professionals as needed.

We seek an enthusiastic, passionate individual to support and manage patients with long-term neurological conditions, requiring community nursing knowledge and some neurological care experience.
The team is based at Warsop Primary Care Centre, covering Mansfield, Ashfield, Newark & Sherwood. Operating hours are Monday-Friday, 8:00 am - 5:00 pm, excluding bank holidays.
The Community Neuro Team includes Specialist Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Psychologists, Assistant Practitioners, and Rehab Assistants.

Salary: £37,338 to £44,962 per annum (pro-rata for part-time)
