Are you looking for a new and exciting challenge? Are you a compassionate nurse or Occupational Therapist (OT) with exceptional forensic, complex, and high-risk mental health skills and experience? We are recruiting two Specialist High Risk and Forensic Practitioners to join our new community mental health Forensic Support Service. Supported by the Reach Out Provider Collaborative, this service will be based across Herefordshire and Worcestershire, improving the interface between specialized services and secondary care community services, enhancing the efficiency of transitions of care for patients with significant risk histories, and improving patient flow through the entire care pathway.
You will offer guidance, recommendations, and advice on complex and high-risk cases, providing an in-reach function to Herefordshire and Worcestershire Health and Care Trust (HWHCT) wards and Neighbourhood Mental Health Teams (NMHTs). Your role will include assessment, advice, and care planning guidance for particularly high-risk individuals. As a reflective practitioner, you will conduct mental health assessments for individuals experiencing complex, high-risk, forensic mental health conditions, including advanced risk assessments and evidence-based interventions. You will work alongside inpatient and community mental health MDTs, striving to ensure the best care for our patients. One post will be based in Worcester, and the other in Hereford. Travel across the two counties is essential, as is flexibility and high organization skills.
