A leading NHS community services provider in Brighton is seeking an experienced SCPHN to join the Healthy Futures Team. This role involves direct engagement with vulnerable families and requires strong public health knowledge, supervisory skills, and a person-centred approach. Key responsibilities include health promotion, community engagement, and liaising with various health professionals. The ideal candidate will hold a SCPHN qualification and have a passion for improving health outcomes.
This is an exciting opportunity for an experienced SCPHN to work in the Healthy Futures Team (HFT) in Brighton and Hove, based in a busy Family Hub in the city centre. The Healthy Futures Team is a commissioned specialist Public Health Visiting and School Nursing service that utilizes early intervention models to improve health outcomes and reduce inequalities for families facing disadvantages in the city. It is a value-led service that employs a person-centred approach, building therapeutic relationships in a relationship-based and trauma-informed way.
The team employs a whole system approach to ensure the best outcomes for families and children. It includes specialist roles working with Teenage Parents, SEND, Emergency Housing, and Women's Refugee services. The postholder will be part of an established, skilled team with access to regular supervision and professional development opportunities.
The postholder will also work with vulnerable transient families within Brighton and Hove, including those in emergency accommodation, asylum seekers, refugees with complex needs, undocumented migrants, Gypsies and Travellers, and those needing refuge from domestic violence.
