Specialist Community Public Health Nurse (SCPHN) qualification

Job description

This is an exciting opportunity for an experienced SCPHN to work in the Healthy Futures Team (HFT) in Brighton and Hove, based in a busy Family Hub in the city centre. The Healthy Futures Team is a commissioned specialist Public Health Visiting and School Nursing service that utilizes early intervention models to improve health outcomes and reduce inequalities for families facing disadvantages in the city. It is a value-led service that employs a person-centred approach, building therapeutic relationships in a relationship-based and trauma-informed way.

The team employs a whole system approach to ensure the best outcomes for families and children. It includes specialist roles working with Teenage Parents, SEND, Emergency Housing, and Women's Refugee services. The postholder will be part of an established, skilled team with access to regular supervision and professional development opportunities.

The postholder will also work with vulnerable transient families within Brighton and Hove, including those in emergency accommodation, asylum seekers, refugees with complex needs, undocumented migrants, Gypsies and Travellers, and those needing refuge from domestic violence.

Responsibilities
  1. The postholder provides supervision and supports the delivery of an effective childhood developmental surveillance and health promotion activities within the organization and with community partners.
  2. Ensures effective liaison and communication with other professional groups within the Trust and outside agencies, acts as a health information source, and works in partnership in the interests of clients.
  3. Identifies community health needs in partnership with health and other agencies, and develops and delivers appropriate services. Plays a significant role in developing public health initiatives for Traveller, BME, and vulnerable transient families, including implementation, monitoring, and evaluation of interventions. Promotes, offers, and administers immunisations, following the routine schedule, at Traveller locations and clinic sessions.
