We have an exciting opportunity for an innovative and forward thinking dietitian to join our community based dietetic service to the proactive care team (PACT) based within our partner organisation Bradford District Care NHS Foundation Trust (BDCFT).
The ProactiveCare Team help to reduce health inequalities and support a reduction in Accident and Emergency Department (AED) attendances and unplanned admissions whilst also improving access to other existing community-based services to help sustain good health outcomes.
PACT are a cross-organisational, culturally aware multi-disciplinary team (MDT) working in Bradford and Keighley to identify those with the highest need and deliver a blend of preventative and recovery focussed interventions in the community: to support a reduction of premature mortality and help deliver improvements for those who are frail and those close to the end of their life.

With the support of the band 7 dietitian team lead you will provide the dietetic service within this MDT and will help to develop the wider MDT to be skilled and knowledgeable in the early identification of key nutritional problems and good nutritional care. Dual based at Horton Park, Bradford and Meridian House, Keighley, you will be required to work within patients homes and across a range of community locations in Bradford and Airedale
The post holder will be expected to participate fully in the training of B and C Placement student dietitians.
We are looking to recruit a dietitian with a broad dietetic knowledge base, excellent communication skills and the ability to manage a demanding caseload.
Applicants must be HCPC registered dietitians, who have obtained relevant clinical dietetic experience, and who are car drivers with access to a car.**
