An exciting opportunity has arisen for a Band 6 specialist dietitian to work in the community Cardiac Rehabilitation service in West Hertfordshire on a permanent contract. We require a highly motivated and skilled dietitian, with excellent communication skills, who is confident in working across professional boundaries to join our enthusiastic multi-disciplinary team. You will need to be flexible, innovative and a good team player with the ability to work independently within the community setting.
The successful individual will be responsible for providing specialist dietetic skills, assessment, and knowledge as part of a multidisciplinary cardiac rehab team across west Hertfordshire and supporting the MDT in delivering cardiac rehabilitation exercise and education session for patients following a recent cardiac event, cardiac procedure or with a diagnosis of heart failure. These assessments and group therapy sessions would take place in a variety of community venues and virtually.
