A regional health service provider seeks a senior triage practitioner to join their 0 to 19 Partnership team in Torquay. The role involves providing expert triage services, supporting families with children and managing health records. Candidates should have experience in child protection and excellent communication skills. This is an office-based position offering the chance to make a significant impact on local families.
The post holder will be a senior member of staff working as a triage practitioner within the integrated 0 to 19 Partnership. They will provide the expertise, knowledge, resource and support to the team managing enquires from pregnant women, families, carers, children and young people in Torbay.
The post holder will provide a comprehensive triage service (for the public and professionals), access and update the Electronic Health record to ensure continuity of care and to make appropriate recommendations and interventions.
They will work with the 0 to 19 Partnership workforces to ensure pathways are followed in supporting children, young people, families and carers.
The service includes the identification of vulnerable children, including those whose welfare and safety may be at risk, working within child protection procedures and in collaboration with other agencies.
The post holder will have the opportunity as part of a team to shape the ongoing development of the role within the Universal Team allocation process and be the Advice line first point of contact for families contacting the Admin Hub.
The post is office based.
