Specialist Clinical Rotational Pharmacist
Working in various specialist areas, the pharmacist will:
- Work closely with Lead Pharmacists, clinicians, nursing staff, and senior managers to develop pharmaceutical services within relevant directorates.
- Provide clinical pharmacy services to specialized areas and cover other areas as needed for ward coverage.
- Assess pharmaceutical care needs, initiate and implement care plans, and conduct medicines reconciliation and optimization reviews.
- Review and select appropriate drug therapies, dosages, and modifications of existing therapies.
- Offer advice to doctors and healthcare staff, participate in discharge planning, and counsel patients.
- Attend ward rounds as required, including post-take and falls review, with flexible start and finish times as needed.
- Ensure services comply with departmental procedures and meet standards set with the Clinical Pharmacy Manager, considering current legislation.
- Collaborate with clinicians and nurses to develop, implement, and monitor protocols to ensure the safe use and supply of medicines, including protocol development and advice on medication safety and storage.
- Participate actively in committees related to the specialist area, such as clinical governance.
- Evaluate, plan, and implement service improvements to enhance the quality of pharmacy services in the specialist area.
