Main duties of the job

The post holder will demonstrate critical thinking, compassion, and commitment, providing expert care within their professional boundaries. Their role includes history-taking, clinical assessment, diagnosis, treatment, and care evaluation, ensuring safe and competent clinical decision-making. They will collaborate with the multidisciplinary team, supporting policies and procedures while offering leadership where required.

This role focuses on preventing unnecessary hospital admissions, enabling patients to remain at home when safe and appropriate. The post holder will proactively identify and prevent relapse or deterioration, reducing admissions to long-term care facilities.

As part of a multi-professional team, they will deliver comprehensive assessments, establish diagnoses, and create evidence-based, person-centred treatment plans, including independent prescribing and medication reviews in collaboration with the medicines optimisation team.

Specialist Clinical Practitioners will help develop and implement effective models of care, ensuring timely clinician-to-clinician advice, supporting patient safety and quality improvement.

Your base will be in Torbay, however under the current COVID-19conditions this may be subject to alternative arrangements/change for examplehome working. Qualifications
  • Clinical based Degree
  • MSc Advanced Practice or working towards or equivalent / minimum of 60 credits at post-graduate level in relevant subject ( e.g. Advanced psychosocial assessment and formulation, Enhancing the Care and Support of the Person with Dementia and their Carer) or equivalent clinical experience
  • Post basic qualification in speciality Application Non-Medical Prescriber (V300)
  • Evidence of Continuous Professional Development
  • Current registration with the NMC or HCPC
