Job summary

NHS Tayside in Scotland is seeking a patient-focused pharmacist to join the Renal Pharmacy Team. The role covers inpatient, outpatient and haemodialysis settings, delivering expert clinical input for patients with chronic kidney disease, those on dialysis and renal transplant recipients.

You will work with consultant nephrologists, specialist nurses and the wider multidisciplinary team to optimise medicines, support complex decision making and contribute to service development and education.

Qualifications

• GPhC registration and independent prescriber status are required.
• Significant post-registration clinical pharmacy experience, ideally including renal medicine.
• Demonstrates excellent clinical, communication and organisational skills.

Responsibilities

• Provide expert clinical pharmacy input to patients with CKD, dialysis and renal transplant recipients.
• Collaborate with consultants, specialist nurses and the multidisciplinary team to optimise medicines and support complex decision making.
• Contribute to service development, education and local prescribing guidelines; participate in governance and quality improvement.

An exciting opportunity has arisen for a patient focused pharmacist to join the Renal Pharmacy Team within NHS Tayside.

This is an excellent opportunity to develop specialist knowledge and skills in renal medicine working as an integral member of the multidisciplinary renal team across inpatient, outpatient and haemodialysis. You will provide expert clinical pharmacy input to patients with chronic kidney disease, those receiving dialysis, and renal transplant recipients ensuring the safe, effective and evidence based use of medicines.

The successful candidate will work closely with consultant nephrologists, specialist nurses and the wider multidisciplinary team to optimise medicines, support complex decision making, contribute to service development and deliver education and training to healthcare professionals. You will also participate in clinical governance activities, quality improvement initiatives and the development of local prescribing guidelines. The Renal Pharmacy Team is part of a bigger clinical pharmacy team and you will have the opportunity to cross cover other specialist areas.

We Are Looking For a Pharmacist Who

• Is registered with GPhC and is an independent prescriber
• Has significant post registration clinical pharmacy experience, ideally including experience in renal medicine or another specialist clinical area.
• Is working towards advanced practice or have experience to an equivalent level.
• Demonstrates excellent clinical, communication and organisational skills.
• Is committed to delivering high quality patient centred care.
• Enjoys working collaboratively within multidisciplinary teams and supporting service improvement.
