A healthcare provider in the UK is seeking a part-time specialist nurse to lead health initiatives for children and young people in Oxfordshire. This role involves monitoring health assessments and addressing health inequalities, alongside collaborative work with various agencies. A rare part-time opportunity has arisen to join the highly regarded Phoenix Team. As a specialist nurse, you will act as a health lead for a defined cohort of children/young people in Oxfordshire to address health inequalities and improve health outcomes for these children.
The defined cohort includes Children We Care For (Looked After Children), CYP in the Youth Justice System (post-court), and CYP at risk of/being exploited.
This is a varied and challenging role providing ample opportunity for professional development and high job satisfaction.
You will undertake and monitor the quality of statutory review health assessments for Children We Care For (Looked After Children), resulting in robust health plans and follow-up actions.
In addition, you will review the health needs of CYP under the Youth Justice Service and those at risk of/being exploited, facilitating access to healthcare and addressing identified health needs.
You will be dynamic and creative in approaching CYP to maximize interventions. You will source and adapt resources and approaches to provide relevant, effective, and evidence-based health education.
Collaborative working is key to this role. You will build robust relationships and offer specialist health advice and support to colleagues within Oxford Health NHS Foundation Trust, Oxfordshire County Council, Social Care staff, foster carers, care providers, residential units, REOC services, and the Youth Justice and Exploitation Service.
You will safeguard CYP at risk of harm through one-to-one work and contribute to multiagency forums including the Missing and Exploited Panel, National Referral Mechanism Panel, MARAMPS, Strategy Discussions, Child Protection Conferences, core groups, Child in Need meetings, CWCF Reviews, and professional meetings. You will be the health representative relevant to the child’s protection and care planning.
