This is a fixed term, full time position until 28/02/26
The shift pattern for this position is 9am-5pm.
The post holder will lead and participate in health needs assessment for children and young people known to Specialist Children’s Services, this will include children with exceptional health care needs, or for whom a request for assistance regarding neuro disability or developmental delay has been received.
Supporting the Clinical Nurse Specialist in managing the effective deployment of nursing resource within the pathways and support clinical functions such as caseload management and supervision; personal development planning, reflecting knowledge and skills framework and the front-line monitoring of implementation of key policy documents.
Supporting the Neurodevelopmental Team Lead and working alongside the multi-professional team to support children and young people with neurodevelopmental differences and their families The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience to lead and enable the SCS Disability and CCN team to provide a quality service.
The post holder will be the clinical expert who will be accountable for case management and SCS Community Children’s Nursing and disability assessments within the caseload
