Adult Speech & Language therapist; Community, Sutton Health & Care, South West London. (Band 6) - 2 positions available.
The Speech & language therapy (SLT) team works across the Community Neuro & Adult SLT service. We offer staff the opportunity to work as part of a Community MDT that supports each other and works towards our vision of 'Investing in our patient community, empowering them to live well'.
Our holistic approach focuses on patient-led goals, providing rehabilitation & intervention to maximize patient potential. We are committed to patient-centered care, using evidence-based practices, and investing in staff training to ensure high-quality service delivery. We utilize clinical expertise and rehabilitation assistants to maximize interventions.
The team works with adult patients with acquired communication & swallowing difficulties. Experience in dysphagia assessment & management, along with rehabilitation experience, is essential. Visits occur in patients' homes, care homes, and clinics, requiring travel around the borough and autonomous working. There is also some remote working, with regular team meetings for clinical support, development, and communication.
The Sutton Community Neuro therapy & Adult Speech & Language therapy service is a multidisciplinary team providing comprehensive assessment & intervention for adults in Sutton with neurological conditions and acquired communication, speech, eating, drinking, and swallowing difficulties.
We offer various pathways to meet patient needs, including intervention for stroke survivors (including Stroke Supported Discharge) and complex neuro conditions like PD, MS, and MND.
Our services provide rehabilitation, enablement, and disability management to promote quality of life & independence, prevent unnecessary hospital stays, and maximize recovery and self-management.
Therapists assess and intervene to educate, train, and support patients and their carers, working flexibly across pathways.

Salary: £42,939 to £50,697 per year, pro-rata, including HCAS (Outer)
