Job summary

Birmingham Community Health Care NHS Foundation Trust is seeking two Band 4 Registered Nurse Associates for the Special School Nursing Service. The roles cover 30 hours per week, Monday to Friday, with responsibilities across Birmingham city schools and community settings.

The posts involve holistic assessment, delivery of clinical care, and coordination of care plans for children and families. Travel across the city to support multiple packages is expected, in a multidisciplinary team.

Qualifications

• Registered Nursing Associate with the NMC or in the process of registering.
• Excellent communication skills, IT competence, adaptability and reliability.
• Well organised with ability to prioritise workload.

Responsibilities

• Undertake holistic assessments of community patients with complex needs.
• Deliver clinical nursing care in school settings.
• Contribute to evaluating and updating person-centred care plans.
• Support families at schools to meet health needs and access Early Help/EDP/SEND plans.
• Support staff in the service and be flexible to travel across Birmingham.
• Be mobile across city-based care packages as required.
