We have an exciting opportunity to appoint highly motivated Health Navigators to join our friendly and supportive Single Point of Access (SPA) team across Bristol, North Somerset, and South Gloucestershire on a permanent basis!

About the Team

Our team consists of 100 Health Navigators & Clinicians working flexibly across 3 sites in Bristol, Clevedon, and Warmley. We offer hybrid working and flexible shifts. The role involves day shifts between 07:30 and 18:00, Monday to Saturday, with a pattern of a full weekend in 6 weeks.

Role Overview

SPA is the first point of contact for all referrals into Planned Nursing, Therapy, and Urgent Community Response. As Health Navigators, you will provide comprehensive administrative support to health and social care professionals, hospitals, GPs, service-users, their relatives, carers, and other stakeholders. You will handle first-line support through phone calls, emails, and electronic referrals.

Candidate Requirements

• Organized with excellent interpersonal, verbal, and written communication skills.
• Detail-oriented with the ability to prioritize and work flexibly.
• Able to work independently to assess patient needs and determine appropriate pathways using clear guidelines.
• Knowledge of administrative processes and practices, including handling queries and problem-solving.
• Ability to liaise effectively with internal colleagues and external agencies, maintaining accurate patient records in Sirona IT systems.
