A leading healthcare provider in the UK is seeking an SPA Clinical Triage Nurse to triage referrals and support the Virtual Frailty Ward. The role involves monitoring the Health Call system and providing telephone support. Candidates should have an RN qualification, 2 years' experience, and strong communication skills. Competitive salary from £38,682 to £46,580 per annum, pro-rata.
The role of the SPA Clinical Triage Nurse is to receive referrals from a diverse range of people and services, triage the referrals, and ensure that they reach the right service in the shortest time. The team also provides significant support to the Virtual Frailty Ward at their daily MDT meetings. SPA Triage monitors the Health Call system, which is utilized by heart failure patients, ensuring that out of range observations are escalated and acted upon. Palcall is a further element of the SPA Triage role; this is a telephone support and signposting service for people deemed to be within their last 12 months of life.
The main duties of the job include receiving referrals, triaging referrals, and providing support to the Virtual Frailty Ward. The team also monitors the Health Call system and provides telephone support through Palcall.
