We are seeking a highly motivated Band 6 nurse to join our busy but rewarding Single Point of Access (SPA) team.

Role Overview

The SPA is a triage service that enables members of the public, GPs, and other healthcare professionals to refer patients to community health services. The successful candidate will use advanced triaging skills to prevent avoidable hospital admissions, deliver high-quality assessments, and manage clinical risks effectively.

Key Responsibilities
  • Perform high-quality triage assessments using enhanced assessment, problem-solving, and decision-making skills.
  • Identify patient needs and appropriate service destinations, including referrals to urgent community response, crisis and social care, community hospitals, and specialist services.
  • Work autonomously and collaboratively within a supportive team environment.
  • Maintain professional development through relevant training.
