About the Role:
Liquid Personnel is seeking an Adult Hospital/Duty Social Worker for our client in Wolverhampton. In this role, you will be part of a fast-paced, multidisciplinary team supporting adults at key transition points in their care journey. Based within hospital settings or operating as part of the duty team, your responsibilities will include facilitating safe and timely hospital discharges, safeguarding, and promoting independence through strengths-based interventions. You will collaborate closely with National Health Service (NHS) professionals, care providers, and community partners to ensure appropriate support is in place – while upholding the principles of the Care Act 2014, personalisation and reablement.
