About the role

In Swansea, we aim to provide adults and their carers with the right support, at the right time, by the right person. Our approach follows the Principles of the Social Services and Well Being Act (Wales), which focuses on working with people, in partnership, to meet the clients' needs and prevent issues from escalating.

We are looking for a dynamic candidate with excellent organizational skills, motivation, passion, and enthusiasm for the role. The successful candidate will work alongside Swansea residents to help them live well and safely in our community.

This is a fast-paced role where you will assess, plan, implement, and evaluate care plans. Your passion for social work will facilitate effective integrated working with vulnerable adults, their families, groups, and communities in Swansea, aiming to promote independence and social inclusion.
