The Isle of Wight Council is recruiting a qualified Social Worker to join the Wellbeing & Access Hub in Newport. The role involves working across Care Act assessments, support planning, safeguarding, and hospital discharge arrangements to help adults and carers live independently and engage with their communities.

You will work with a supportive team, receive supervision, and have opportunities for professional development. Regular caseloads and a focus on person-centred outcomes are expected.

Qualifications

• Qualified and registered Social Worker.

Responsabilités

• Work across Care Act assessments, support planning, reviews, safeguarding, risk management, and hospital discharge arrangements.
• Collaborate with individuals, carers, health colleagues and community partners to promote wellbeing, independence and positive outcomes.
• Provide regular supervision, ongoing professional development and support within a collaborative team.
• Manage a diverse caseload and contribute to shaping services for Island residents.
