Social Worker - Transitions Team - Woking

Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for a Social Worker within the Transitions Team to work full-time based in Surrey, Woking.

The salary for this permanent Social Worker job is up to £39,246 per annum.

Responsibilities:
  1. Assess and/or coordinate the assessment of the strengths and needs of users and their primary carers using the established assessment format, within the department's criteria and legislative framework.
  2. Develop and cost flexible, cost-effective care packages responsive to changing needs, promoting independence and choice through access to services from both independent and statutory sectors.
  3. Undertake specialist social work care planning and provide direct social work interventions.
  4. Contribute to managing the adult care budget, ensuring expenditure remains within set limits.
  5. Ensure individuals receive information in accessible formats to enable informed choices.
  6. Work actively in partnership with other agencies.
  7. Review and coordinate care packages regularly, making necessary adjustments.
  8. Identify service deficiencies and communicate these to the line manager.
  9. Participate in relevant training opportunities.
  10. After two years post-qualification, undertake to become a practice assessor and/or facilitator for social work students on placement.
Requirements:
  • A social work qualification or equivalent.
  • Understanding of relevant legislation.
  • Registered with Social Work England.
