Social Worker - Transitions Team

Location: Colindale, London (Hybrid)

Salary: £45,510 per annum (Grade I)

Contract: Full-time, 36 hours per week

About the Transitions Team

The team supports young people with disabilities transitioning from children's to adult services, ensuring they receive the necessary care from ages 18 to 25. We engage with them from age 16, working with schools, colleges, and social care teams.


The Role
  1. Assess and plan care for adults with mental health or physical disabilities.
  2. Work with Barnet, Enfield and Haringey Mental Health Trust and other partners.
  3. Conduct strengths-based assessments and person-centred planning.
  4. Undertake statutory responsibilities under the Care Act and Mental Health Act.
  5. Lead complex assessments and safeguarding enquiries.

About You
  1. Qualified Social Worker registered with Social Work England.
  2. Experience in social work with vulnerable adults.
  3. Strong assessment, report-writing, and case management skills.
