Social Worker - Transition 16-25, Sutton

We have an exciting opportunity for you as a Social Worker in Adult Social Care. Our mission is to empower young adults to live independently, and we need your expertise to make it happen. This role offers a dynamic and diverse environment to expand your skills. Your duties will include:

  • Conducting Care Act compliant eligibility assessments for young people and their carers, identifying social care needs, and developing person-centred, strengths-based support plans.
  • Monitoring and reviewing support plans to ensure they meet changing needs and reflect good value for money, advocating for young people's rights and access to services.
  • Safeguarding and promoting the welfare of young people, assessing risks and needs, and undertaking safeguarding investigations as necessary.
  • Managing a complex caseload, making professional judgments with appropriate supervision.
  • Identifying young people with complex health needs and initiating the Continuing Healthcare Process for NHS funding eligibility.
  • Collaborating with multidisciplinary teams to meet the holistic needs of young people.

As a Social Worker within the Transition Team, you will be the first point of contact for young people in Sutton, supporting their journey into adulthood by working closely with them, their carers, and other services.

Within our boroughwide service, there are three social work teams: The Transition Team, The Learning Disability Team, and The Mental Health Team. The Transition Team prides itself on supporting staff development through comprehensive induction, ongoing training, supervision, and flexible working arrangements, including a hybrid model of working.

About You

This role is ideal for individuals with excellent communication skills and a compassionate approach, capable of managing a fast-paced workload. We welcome applications from those experienced in adult social care and passionate about person-centred care and risk management.

Candidates should have:

  • Qualified Social Worker registration with Social Work England and eligibility to work in the UK.
  • At least 2 years post-qualifying experience in adult services, with assessment and support skills, and successful completion of the ASYE.
  • Knowledge of relevant legislation such as the Care Act and Mental Capacity Act.
  • Experience in adult safeguarding and outcome-focused care planning.
  • Ability to travel across the borough for visits, including occasional out-of-borough visits.
