Job Description:

Pertemps Social Care is currently recruiting for an experienced Social Worker to join a busy team in Tower Hamlets, within the Adults Social Care team.

Responsibilities:

• Provide a professional social work service to adults, their families, and carers in line with the requirements of the Care Act 2014, focusing on promoting wellbeing and identifying significant impact.
• Comply with statutory requirements of relevant legislation, including mental health legislation, and apply the Mental Capacity Act 2005 to assess and manage risks.
• Assist individuals, carers, and families by completing strength-based assessments to identify personal, community, and universal strengths, supporting evidence-based eligibility decisions and promoting value.
• Develop support plans that meet personalized outcomes, taking a preventative approach to reduce and delay needs, and providing advice and information to support informed decision-making, including arranging advocacy as appropriate. Work holistically with all relevant professionals.
• Undertake safeguarding enquiries in accordance with legislation, guidance, and best practices, ensuring the process is person-centered and safeguarding is made personal.
