An established industry player is seeking a dedicated social worker to join their Integrated Discharge Hub. This role is pivotal in facilitating safe discharges from the hospital, ensuring that patients receive the support they need. The successful candidate will be based within the Integrated Discharge Hub - Hospital Social Work Team and responsible for completing assessments for adults and older people to facilitate safe and supported discharges from hospital.
The Integrated Discharge Hub is an inter-agency, multi-disciplinary team comprising hospital social workers, community care officers, reablement staff, discharge facilitators, AHP staff, and other representatives from various services. The hub promotes a 'home first' approach and aims for patients to be discharged on their predicted date of discharge (PDD).
This is an exciting opportunity for the right person to collaborate with a range of professionals and service users to support the principles of Home First.
The worker must have the ability to work as part of a team and respond proactively to the demands of a busy service, identifying opportunities to promote Home First. They must follow policies and procedures, possess excellent communication skills, and have developed assessment and report writing skills. The worker will be required to provide daily updates on their caseload related to discharge planning.
Tasks will be carried out in partnership with service users, family members, colleagues within Social Policy, and other professionals and agencies as necessary to meet needs. A good understanding of Self-Directed Support and the importance of completing outcome-focused assessments is essential.
The worker must have the ability to travel throughout West Lothian.
Before confirming appointment, candidates will be required to obtain PVG Scheme Membership through Disclosure Scotland.

This position requires registration with the SSSC. The successful candidate will have six months from the start date to register with the SSSC. Applications must be submitted within three months of starting. If already registered with the SSSC, candidates must add West Lothian Council to their account.
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will need to join the PVG Scheme or undergo a PVG Scheme update check before a formal offer of employment is made by West Lothian Council.
