Overview
Are you a social worker with experience in a mental health setting, looking for a new challenge? The Langford Centre is a purpose-built facility providing care for individuals with complex mental health needs, including dementia and neurodegenerative disorders. Across our 6 wards we provide low secure, high dependency inpatient rehabilitation and acute services, supporting patients from admission to engage in their recovery pathway and promote successful reintegration into the community. Sponsorship is available for this role for candidates based both inside and outside of the UK.
We have an opportunity for a Social Worker to join our team. Working alongside a multidisciplinary team, you will have clinical responsibility for in-patients while providing support to patients and their families. This is achieved through a blend of treatment and rehabilitation during the stay at the Langford Centre. The role also involves safeguarding, training, and functioning within the Care Programme Approach.
Responsibilities
- Assessment of patient needs for social work interventions throughout admission including 1-1 and group work with patients.
- Ensure patients\' rights are protected under the Mental Health Act 1983, Human Rights Act, Safeguarding of Vulnerable Adults (SOVA), and Community Care legislation.
- Advocate for patients or ensure involvement of appropriate advocacy services.
- Prepare and present Mental Health Tribunal and social history reports.
- Work with carers and family members including visits to family homes, liaison with health and social care services, other voluntary or statutory agencies, and identifying and assessing appropriate community resources.
- Plan for discharge and record Section 117 aftercare arrangements.
- Attend CPA meetings and present evidence of social work intervention.
- Complete clinical audits and prepare reports.
- Carry out capacity assessments.
- Prepare monthly reports for Corporate Clinical Governance meetings.
- Take responsibility for safeguarding issues, including being the main point of contact for advice and referrals for safeguarding.
- Undertake home visits and complete relevant history/risk assessments for the safe management of patients with forensic history as required.
- Identify, visit and assess resources appropriate for service users moving into the community or other residential settings.
- Maintain up-to-date records in clinical notes of all service user contacts and observe patient confidentiality, other than risks agreed by the clinical team.
Qualifications
- Verifiable experience of working as a Social Worker in a similar setting.
- Experience of working with individuals with mental health issues and/or forensic history and their family/carers in hospital and/or community settings.
- Experience of working with people with challenging behaviours.
- Willingness and ability to engage in further education and training relevant to social work practice and secure conditions.
- Willingness to develop an understanding and knowledge of managing patients with a forensic history.
