Looking to Appoint as a Level 2 or Level 3 Social Worker.

The Specialist Adult Pathway Team/SAPT sits alongside the Community Learning Disability Team as part of the All Age Disability Team. The role of SAPT is to work with young people aged 14-25 years and their families with a focus on preparing them for adulthood. This involves close liaison with the Children with Disabilities service and other Children’s Social Care teams. In preparing the young person for adulthood, the Social Worker will undertake an holistic strength-based assessment and to develop personalised support plans to meet the young persons needs and outcomes, in preparation for them turning 18.

You will work collaboratively with a range of professionals (health care providers, education, housing etc) to coordinate care and ensure seamless service delivery.

Our team works with service users, their families, and carers to enable them to be independent, fulfilled and to live within their communities.

We are looking for dedicated, hardworking, person centred Social Workers who are committed to developing high quality strength based packages of care and support , whom have experience of working with Adults and/or Children with disabilities, including multi-disciplinary working, knowledge of the Care Act, Deprivation of Liberty Safeguards, Mental Capacity Act, Special Education Needs and Disabilities (SEND) legislation, Personalisation and delivering a high quality service to our service users and carers.
