Job Title: Social Worker – Southwark – All Age Disability Service (0-25)

Rate: £32.00

Liquid Personnel is currently recruiting for a Social Worker to join a busy All Age Disability Service (0-25) based in Southwark.

Responsibilities:

  • Provide comprehensive, effective social work services for children and young people with disabilities, in line with legislation and departmental policies.
  • Undertake strength-based assessments and develop outcome-focused support plans.
  • Deliver interventions to safeguard children and young people, promote independence, and encourage assistive technology use.
  • Support seamless transition for children and young people with disabilities from childhood through adulthood.

Qualifications and Experience:

  • Qualified Social Worker with HCPC registration
