What will your responsibilities be?
As a Social Worker in the Adult Community Team based in Sefton, you will be responsible for managing a diverse caseload, conducting assessments, and developing care plans to support adults in the community. You will work collaboratively with other professionals to ensure the safety and well-being of service users, addressing their needs through a person-centred approach. Additionally, you will be expected to handle complex cases, provide crisis intervention, and ensure compliance with relevant legislation and policies. Your role will also involve advocating for service users, promoting their independence, and maintaining accurate records of all interactions and interventions.
