Job Description:

An exciting role has become available at Sheffield City Council for their Adult’s Mental Health Hospital Discharge Team.

Responsibilities include:

  • Undertaking social care assessments and delivering care packages for hospital inpatients prior to discharge.
  • Conducting mental capacity assessments, making best interest decisions, and handling safeguarding as needed.
  • Attending 117 meetings, ward rounds, and professional meetings.
  • Facilitating learning opportunities for ASYEs through shadowing and participating in the duty rota.
  • Receiving regular supervision and support as part of the team.

Qualifications and Experience:

Must have Social Work England registration, a full UK driving licence, post-qualified experience, and a degree or equivalent in Social Work.
