Job Description:

An exciting opportunity has become available for a Qualified Social Worker in the Learning Disability & Autism Team within Adult Social Care.

Responsibilities include:

• Managing the transition allocation list and addressing referral backlogs to support timely assessments for young people transitioning to Adult Services.
• Following a hybrid working model with a mix of office and home-based work.
• Handling complex cases, including risk assessments and management, as a Level 3 Social Worker.
• Providing peer guidance through knowledge sharing and expertise.
• Overseeing social work student placements and possibly providing court evidence for care proceedings.

Qualifications and Experience:
