Job Title: Social Worker – Sefton – Hospital Team

Location: Sefton
Rate: £35.00 per hour

An exciting job role has recently become available at Sefton as we are looking for a Qualified Social Worker in the Hospital Team.

What will your responsibilities be?

In this role, you will conduct assessments and develop discharge plans for patients transitioning from hospital to home or other care settings. You will coordinate with healthcare professionals, families, and community resources to ensure a smooth and safe discharge process. Additionally, you will provide support and advocacy for patients and their families, addressing any concerns or needs. Monitoring and reviewing care plans to ensure they meet the ongoing needs of patients is also a key responsibility. You will maintain accurate and detailed records of all assessments, plans, and interactions, ensuring compliance with relevant legislation, policies, and procedures. Participation in team meetings and contributing to the continuous improvement of the Hospital Team’s services are also essential aspects of this role.

Qualifications and Experiences:

You must have Social Work England registration, be eligible to work in the UK, and hold a full UK driving licence. Post-qualified experience and a degree level or equivalent qualification in Social Work are also required.
