What you will do:
- Engage directly with individuals, carers, and families to undertake comprehensive strength-based and asset-based assessments in accordance with statutory requirements.
- Utilise your professional judgement and a range of social work interventions to assist individuals in achieving positive outcomes.
- Develop, implement, and review outcome-focused support plans, ensuring co-production with the individual and their support network.
- Encourage and enable choice, control, and independence for service users, leveraging community assets and informal networks whenever possible.
- Provide clear and accurate information on Personal Budgets and Direct Payments, aiding individuals in making informed decisions about their care.
- Collaborate with other professionals, including health, housing, voluntary services, and key partners, to deliver integrated support.
- Identify, assess, and address safeguarding concerns in line with local procedures and legislative frameworks, including the Care Act 2014.
- Lead complex cases or those with high risk, providing expert advice and guidance to less experienced colleagues where necessary.
- Maintain precise, up-to-date case records, ensuring compliance with data protection and professional standards.
- Contribute to team development, practice improvement, and service transformation activities.
Minimum Requirements:
- Degree or equivalent qualification in Social Work.
- Registration with Social Work England as a qualified Social Worker.
- Demonstrated experience with adults with learning disabilities and/or mental health needs.
- Strong knowledge of relevant legislation, policies, and procedures, such as the Care Act 2014 and safeguarding frameworks.
- Ability to work independently while upholding high professional standards and reflective practice.
- Excellent communication, interpersonal, and organisational skills.
- Proven ability to develop effective support plans and promote meaningful co-production.
- Experience in risk assessment and management in complex cases.
- Familiarity with case management systems like Mosaic or Liquidlogic.
- Understanding of local authority processes and commissioning frameworks.
- Ability to facilitate multi-agency meetings and case conferences.
