Job Title: Social Worker – Rotherham – Transitions Team

Location: Rotherham

Rate: £32.99

An exciting job role has recently become available at the Rotherham Metropolitan Borough Council. They are looking for a new member for their Transitions Team.

Responsibilities include:

  • Supporting young people with disabilities transitioning from children’s to adult services
  • Conducting comprehensive assessments to identify needs and interventions
  • Developing personalized transition plans
  • Providing emotional support and counselling
  • Coordinating resources and services
  • Monitoring and evaluating client progress
  • Facilitating communication between clients and relevant parties
  • Assisting with job placement or educational transitions
  • Advocating for clients’ needs and rights
  • Organizing and conducting workshops or training sessions
