Job Title: Qualified social worker
Location: Rhondda Cynon Taff
Rate: £38 per hour
An exciting opportunity has recently become available for a qualified Social Worker at the Rhondda Cynon Taf Council. They are looking for a new member to join the Adults’ Hospital Discharge team, offering a pay rate of £38 per hour.
What will your responsibilities be?
Understanding of the hospital discharge process and working within a multi-disciplinary setting, as well as an understanding of the NHS Continuing Health Care framework in Wales, and the principles and legislative framework of the Mental Capacity Act 2005. More importantly, you should be positive, enthusiastic and a supportive Social Worker who can work with individuals, families and carers in line with the duties and principles of the Social Services and Well-being (Wales) Act 2014 and contribute to the development of high quality integrated working practices to ensure citizens are discharged safely from hospital in partnership with our colleagues across Health and Social Care.

Qualifications and Experiences:
Candidates must have SCW registration and be eligible to work in the UK. They should have completed ASYE or possess equivalent demonstrable experience in a statutory social work setting. Additionally, candidates must hold a valid UK driving license and have access to a car, with exceptions potentially applicable under the Equality Act. A degree level or equivalent in Social Work is also required.
