An exciting opportunity for an experienced Hospital Social Worker is sought for the Adult Services team at Rhondda Cynon Taf County Borough Council. The role is central to the council’s commitment to safeguarding and promoting the welfare of vulnerable adults within the hospital setting.
What will your responsibilities be?
As a Social Worker, you will conduct in-depth assessments to determine the needs of adult service users, craft and revise care plans in partnership with service users, their relatives, and other healthcare professionals, and advocate for service users to ensure their needs and voices are central to care decisions. The ideal candidate will be an integral part of the multidisciplinary team, ensuring that social care is seamlessly integrated with medical care. Additionally, you will be expected to keep precise records and engage in continuous professional development.

A recognised qualification in Social Work, along with registration with Social Care Wales and DBS, and considerable experience in social work, specifically within a hospital context, are essential.
