Pay Rate: £33.76 per hour

Liquid Personnel is currently recruiting for an Adult Social Worker to join a Reablement Team based in Southwark.

About the role

Seeking a Social Worker to work within the Reablement Service. This role offers a unique opportunity to thrive in a multi-disciplinary environment, collaborating closely with health professionals.

What will your responsibilities be?

1. Goal setting
2. Completing Care Act assessments and devising care and support plans
3. Completing reviews
4. Completing Mental Capacity Assessments
5. Chairing Best Interests meetings

Qualifications and Experience:

To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a Social Work degree, and have post-qualified experience.
