What will your responsibilities be?
In this role, you will conduct comprehensive assessments of young people’s needs and create personalized transition plans. You will manage a caseload of young adults, coordinating care across services and agencies, while advocating for their rights and ensuring access to appropriate support. Working closely with multidisciplinary teams—including healthcare professionals, educators, and local authorities—you will deliver integrated care. You will also provide emotional and practical support to young people and their families during transitions, ensuring continuity and stability. All responsibilities will be carried out in line with the Children and Families Act (CFA) and relevant safeguarding legislation.
