You will provide ongoing Social Work assessment, support planning and interventions for Adults aged 18+ with a range of complex needs in the Darlington area. We are based at The Town Hall in Darlington.
The team works proactively with service users, their families, carers and professionals to maximise independence through a comprehensive strengths-based approach to assessment and support planning. The dynamic nature of the team is underpinned by a strong commitment to the principles of The Care Act 2014 and other relevant legislation.
The successful applicant will be required to contribute to the operational delivery of an effective Adult Social Care Service and be responsible for a range of assessments including Care and Support Needs, Mental Capacity Act, Safeguarding, and Support Planning to promote the wellbeing of adults in line with the Care Act 2014.

We are looking for both newly qualified and experienced social workers who are committed to strength-based practice, focusing on enabling people to build on their own networks and strengths as a means of meeting their needs.
