Job Title: Social Worker – Older People’s Mental Health (OPMH) Team

Location: Bath and North East Somerset Council

Pay rate: Up to £38 per hour

The Bath and North Somerset Council is seeking an experienced, hardworking, and reliable Social Worker to join their Older People’s Mental Health (OPMH) team.

Responsibilities include:

  • Providing outcome-focused assessments to adults in line with the Care Act 2014.
  • Delivering professional social work services, including statutory and legal responsibilities.
  • Working closely with other teams, undertaking joint assessments with providers, carers, and external agencies.
  • Coordinating comprehensive holistic assessments of needs and risks.
  • Participating in safeguarding procedures and complying with departmental policies and the HCPC’s Code of Conduct.

Qualifications and Experience:
