Rochdale Adult Care is seeking a Qualified Social Worker to join the Home First Hospital Discharge Hub. The role involves working with adults and their carers, promoting independence, and collaborating with health colleagues. Applicants must demonstrate the ability to travel around Rochdale Borough and to various hospital sites. The role involves a seven-day working rota.
