Job Overview

Rochdale Adult Care offers an opportunity for a positive, enthusiastic Qualified Social Worker registered with Social Work England to join the Home First Hospital Discharge Hub, based within the Integrated Middleton Locality.

Role Requirements
  • Must be a confident, dynamic individual with excellent communication skills, capable of working in a fast-paced environment.
  • Experience and motivation to work with adults, their carers, and develop positive relationships with health colleagues across hospital sites.
  • Ability to work within timescales and deadlines.

  • Excellent knowledge of the Mental Capacity Act and safeguarding within an Adult Care setting.

Travel and Working Conditions

Applicants must demonstrate the ability to travel around Rochdale Borough and to various hospital sites. The role involves a seven-day working rota.

Interview and Conditions
