We seek dedicated, motivated team players passionate about improving outcomes for adults, capable of building trust with complex customers, and experienced in mental health assessments, Care Act assessments, and developing support plans.

The role involves managing a high caseload of vulnerable individuals with complex mental and physical health issues, often with dual diagnoses, and advocating for their needs. We support our staff with training, career development, and a values-driven culture.

Key responsibilities include:

  • Supporting adults with care and support needs to achieve their outcomes
  • Connecting individuals to appropriate services and community assets
  • Engaging with complex and vulnerable clients to build trust
  • Conducting mental health and Care Act assessments
  • Developing and implementing support plans

Requirements include:

  • AMHP qualification (postgraduate certificate or master's level)
  • Experience managing high-risk, complex cases and safeguarding adults
  • Knowledge of mental health legislation
  • Ability to work autonomously and effectively communicate
  • Enhanced DBS check and Adults Barred List clearance

This hybrid role offers flexible working from Council offices and home, along with benefits such as discounts, generous holiday allowance, pension scheme, training opportunities, and wellbeing support.
