Responsibilities
  • Undertake comprehensive social care assessments for hospital inpatients to ensure they receive timely care packages before discharge.
  • Conduct mental capacity assessments and make best interest decisions, participating in safeguarding processes when necessary.
  • Engage in Section 117 reviews and attend related meetings, professional gatherings, and ward rounds.
  • Participate in the duty rota, ensuring continuous support for the hospital discharge team.
  • Preferably have experience with LAS (Local Authority Software) and be able to navigate its complexities with ease.
  • A car driver’s license is essential to facilitate on-site assessments and meetings efficiently.
